What the request form collects
The forms ask for your name and email, and optionally your phone number, your ZIP code or area, the procedures you are curious about, when you are thinking of surgery, how you would like to be contacted and a short message. The forms do not ask for, and you should not send, medical history, medicines, health conditions, photographs, your date of birth, ID numbers or payment details.
Your consent
Every form has a consent box that is never ticked in advance. By ticking it you agree that Mommy Makeover Dallas may share your request with one or more independent providers in your area so they can contact you by email or phone about it. A record of the request and of your consent, with the date and time, is kept to show what you agreed to.
Who your request is shared with
